We have a critical need on our Transitions of Care (TOC) Clinical Information Model (CIM)/Vocabulary Workgroup for knowledge on data element mappings, specifically surrounding work done on the HITSP C154, also known as the HITSP Data Dictionary.
We want to be able to define the key care transition data elements for our four Data Element Groups deemed as critical to building our TOC CIM:
2. Allergies and Adverse Reactions (Medication)
3. Active Problem List
4. Reconciled Medications
HITSP C154 expertise would be important to us as the C154 serves as an important reference source to use in determining what these key elements would be. We can also leverage general overall data element expertise in these areas (including participation from experts in the Clinical Care Classification (CCC), the Clinical Care Record (CCR) and the Continuity of Care Document (CCD))
